Care Teams are available 24/7 via scheduled or on-demand virtual visits, calls and/or in-app messaging — and can facilitate in-person care when needed.
How the plan works
It begins when your employee needs care. They start a virtual visit with their Care Team via the Optum Virtual Care app.
When they enroll
They’ll download the Optum Virtual Care app so they can connect to care — anytime, anywhere. Your employee should also provide their cell phone number and email address to get important plan updates.
After they enroll
They'll be matched with a dedicated Care Team — including a PCP — based on location, preferences and health profile. Their Care Team will provide ongoing care virtually and guide them to in-person care when they need it.

NavigateNOW is available in select areas, so you’ll need to check your eligibility. After you enroll, download the Optum Virtual Care app. When you need care, you’ll begin with a virtual visit with your Care Team via the Optum Virtual Care app. It’s a good idea to download the app now — so you’ll be ready to connect to care when you need it.
A paper referral won’t be considered a valid referral — so your Care Team/PCP must submit all referrals electronically. You can check your referrals online at myuhc.com®.

NavigateNOW is a new virtual-first health plan that offers the convenience of a single entry point via the Optum Virtual Care app or web portal to medical, behavioral and pharmaceutical services that begin virtually and move to in-person when necessary. It’s a health plan that’s built to help seamlessly guide you to care — when and where you need it — through a personal Care Team.
Your personal Care Team, which includes a primary care provider (PCP), is provided by Optum Virtual Care and is matched to you based on your location, preferences and health profile. Your Care Team may include board-certified physicians, nurse practitioners, care navigators and other providers who can guide you to primary, urgent or behavioral care — when and where you might need it. Your Care Team will be available 24/7 via scheduled or on-demand virtual visits, calls and/or in-app messaging to help you achieve your best health. Once assigned, you may select a specific Care Team member to engage with.
Your care journey will start with a virtual visit with a member of your Care Team via the Optum Virtual Care app or web portal. You may be guided to in-person care as needed. You will have 24/7 access to care and support — plus online scheduling and on-demand, same-day virtual appointments at the touch of a button.
NavigateNOW provides a holistic approach to care and includes primary care, wellness and behavioral support. Your Care Team can address a variety of clinical issues and questions, such as:
If you need to be seen in person, your Care Team will send the required information to the in-person clinic and/or physician and track the completion and outcome of your in-person visit. In addition, you will receive a post-visit summary after your virtual visit detailing next steps.
You and covered dependents will be matched with an Optum Virtual Care Team — which includes a PCP — based on your health profile. You are required to visit your virtual Care Team for primary care and obtain referrals to other providers and sites of care. Each family member may select a different PCP-led Care Team, if desired. All dependents must reside in the subscriber’s service area; this includes students going to school out of state or children living with another parent. If you have a medical emergency, you will be covered for emergency care received in any hospital emergency room facility. If you require nonemergency medical attention, you also have the option to seek care through any Navigate network urgent care or convenience care clinic (without a referral from your PCP). You can also get a referral from your PCP to see a specialist or other network provider in the network service area where you or covered dependents are attending school or traveling.
Yes, you can change your virtual PCP any time through the Optum Virtual Care app or web portal.
Yes, minor children or dependents are eligible for this plan and Optum Virtual Care is available to members of all ages. If you have dependents under the age of 18, you can switch from a virtual Care Team to an in-person pediatrician for your minor dependent. To change from the Care Team to a pediatrician, call the UnitedHealthcare customer service number on your health plan ID card and request a pediatrician for your minor dependent.
The first time you or a covered dependent use your NavigateNOW plan, you will complete a short patient profile that includes your medical history and preferences. Then, you’ll schedule a virtual visit using secure video with a Care Team provider who is licensed to deliver care in your state. Your provider will ask about your health concerns and may write a prescription. The details of your visit will be documented and available to you within your digital records.
NavigateNOW requires a referral to be obtained from your virtual Care Team/PCP prior to seeing another PCP or specialist. Referral requirements will be administered like any other PCP-led plan, and the Care Team (led by the physician) will provide referrals as needed based on the benefit plan.
